Mailing Exposes Personal Data of Wisconsin Residents

January 14, 2008 (Computerworld) -- The Wisconsin Department of Health and Family Services last week called on Electronic Data Systems Corp. to explain how Social Security numbers were printed on the address labels of brochures sent to more than 260,000 recipients of state health services.

The state agency is asking the Plano, Texas, outsourcer to explain the gaffe, inform the affected individuals of the error and provide each with a year of credit-monitoring services.

“It was clearly an error on EDS’s part,” said Karen Timberlake, deputy secretary of the state agency. “They need to explain to us what happened and what they are doing to ensure this doesn’t happen again.”

An EDS spokesman called the incident an isolated error that occurred when the address file was created earlier this month. The company plans to take “appropriate actions” against those involved, he added.
